A Behaviour Support Plan is an elaborate document which is created by our NDIS registered Behavior Support Practitioners. Our practitioners collaborate with the NDIS participants, their family members and other relevant support networks to establish an individualized approach to target any Behaviors of concern. The plan acts as a tool to guide the participant’s family members, providers, and any other support networks on how to appropriately support the participant when a behavior of concern arises. A behavior support plan will also contain various strategies to aid the participant, with the aim of reducing the frequency of the behavior in the future, as well as any potential harm posed by the behavior. For example: A behavior plan may be provided to a participant’s school teacher if behaviors are present during class time.
The team at Towards the Horizon are made up of passionate and dedicated practitioners who specialise in a variety of different health domains. All practitioners have extensive knowledge and experience working with the NDIS, and value the importance of equality and autonomy of each participant living with a disability. The roles of a Behavior Support Practitioner include tasks such as researching, completing questionnaires and assessments with the participant, frequent meetings with the participant’s care team (family members/guardian’s, therapists, employers, school teachers), report writing and so on.
Once a Behavior Support Plan has been implemented, the plan will be closely monitored by the practitioner to assess how well it works. What may work for one individual, may not for another. By using a person-centred approach when building a Behavior Support Plan, the outcome of a positive result increases as well as the quality of support delivered for the participant.
